FAQs

The Student Health Center is a clinic located at the Hermiston High School campus operated by qualified health professionals.

The Student Health Center team includes a nurse practitioner and office assistant.

Appointments are encouraged whenever possible. Appointments can be made at the Student Health Center or by calling the center at 541-667-6199.

Our health services and records are private, confidential and separate from school records.

All students need to be registered with the Student Health Center each school year before services will be provided. To register forms can be found on this webpage, picked up in person or mailed. They can be returned to the Student Health Center by mail, fax, or in person.

requires a parent or guardian signature for mental health services for students under 14 years of age and medical treatment under 15 years of age, with exception of reproductive health and sexually transmitted disease service. We support and encourage parental involvement in decisions about their student's health care.
